Registration Requirements
for
New, Transfer, and Re-entry Students

    * ALL STUDENTS MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN.

** NO STUDENT WILL BE ENROLLED WITHOUT THE FOLLOWING DOCUMENTATION:

• Withdrawal form
  Proof of prior school enrollment
• Attendance Report
  From former school
  
• Transcript
  A certified/official copy of his/her academic transcript listing classes, grades received and credit earned.
  
• Discipline Records
  A certified/official copy of all discipline records.  If there have been no rules infractions, student must present a statement from the previous school to that effect.
  
• Copy of Birth Certificate
  New students only
  
• Copy of Social Security Card
  
• Proof of Residency
  The parent/guardian MUST provide current lease and/or closing statement in the case of a home purchase or current utility bill (in parent's or legal guardian's name) including gas, electric, cable, telephone or water.  This MUST be provided at the time of registration.  NO driver's license, checkbooks, insurance forms, or tax forms.
  
• Custody/Guardianship documentation:   Proof of custody/guardianship MUST be provided if the child does not live with one or both parents.  This document MUST be provided at the time of registration.  If the parent is not the custodial parent, guardianship papers from the probate court are required.
  
• Immunization Form:   All students must submit a Georgia Certification of Immunization (Form 3231 or Form 3032) at the time of registration.  Form 3231 will be for students entering a GA school for the 1st time.
  Georgia School - In permanent folder at previous school
  Out-of-State school - Take Out of state records to the health department in Georgia to be transferred to the Georgia Form.
  
• Eye, Ear, and Dental form:   All students entering a Georgia Public School for the first time must have a Georgia DHR Form 3300 that has been completed within the previous 120 calendar days of registration (out-of -state forms will be accepted if dated within the previous 12 months).
